Deep cleaning in San Antonio: what scaling and root planing is, what it costs, and how it feels

You came in for a cleaning and heard the words "deep cleaning" instead. That can feel confusing, and maybe a little worrying. Here's what a deep cleaning really is, why it's recommended, what it feels like, and what it costs in San Antonio, in plain language, from Dr. Parveen Bhatti at Serenity Oaks Dental in North Central San Antonio (78247).

What a deep cleaning is

A deep cleaning is the everyday name for scaling and root planing (often shortened to SRP). It's the standard first treatment for gum disease, and one of the most common treatments we provide for patients across San Antonio.

  • Scaling removes hardened plaque (tartar) and bacteria from your teeth, including from the pockets below the gumline where a toothbrush, floss and a regular cleaning can't reach.
  • Root planing smooths the root surfaces so bacteria have a harder time sticking, and your gums can heal and tighten back around the teeth.

The mouth is usually treated in four sections called quadrants (upper right, upper left, lower right, lower left). Many patients have two quadrants done per visit, so the whole mouth takes two visits.

Deep cleaning vs. regular cleaning

  • A regular cleaning (prophylaxis) is preventive. It cleans the teeth above and just at the gumline when gums are healthy.
  • A deep cleaning is treatment. It goes below the gumline to clean infected pockets when gum disease has already started.

Think of it like this: a regular cleaning keeps healthy gums healthy. A deep cleaning is what helps unhealthy gums get healthy again.

Why you might need one

Gum disease is very common in Texas and across the country. The CDC estimates that nearly half of U.S. adults age 30 and older have some form of it.

At your exam, we gently measure the space between each tooth and the gum, called a pocket. Healthy pockets are usually 1 to 3 millimeters deep. When pockets measure about 4 millimeters or more, and your X-rays show tartar under the gums or early bone loss, a regular cleaning can't reach the problem. That's when a deep cleaning is recommended.

Signs you might notice at home:

  • Gums that bleed when you brush or floss
  • Red, puffy or tender gums
  • Bad breath or a bad taste that doesn't go away
  • Gums pulling back, making teeth look longer
  • Teeth that feel loose or a bite that feels different

Gum disease usually doesn't hurt in the early stages, which is why many people don't know they have it. Left untreated, it's a leading cause of tooth loss in adults, and it has been linked with other health conditions such as diabetes and heart disease.

Does a deep cleaning hurt?

Most patients are surprised by how comfortable it is. We numb the area first, just like for a filling, so you feel pressure and vibration but not pain. We work one section at a time and check in with you often.

If you're nervous, tell us. We'll go at your pace, explain each step, and you can raise your hand for a break anytime. See what a gentle visit looks like at our office.

How much does a deep cleaning cost in San Antonio?

Deep cleanings are priced per quadrant, because not every section of the mouth always needs it.

  • Without insurance: in San Antonio and across the U.S., scaling and root planing commonly costs about $200 to $400 per quadrant, or roughly $800 to $1,600 for the full mouth. The exam and X-rays are usually separate.
  • With insurance: many dental PPO plans cover deep cleanings at about 50% to 80% after your deductible, depending on whether your plan lists it as a basic or major service. Your annual maximum and any waiting periods also apply.

Your price depends on how many quadrants need treatment and how deep the pockets are. We always give you a written estimate before we begin, and we'll check your insurance benefits for you. See the insurance plans we work with.

No insurance? No problem.

Is it worth it? The cost of waiting

Gum disease doesn't go away on its own. As pockets deepen, the bone that holds your teeth in place breaks down. We see it often in San Antonio patients who put off care for a few years: waiting can turn a deep cleaning into gum surgery, extractions or tooth replacement, all of which cost far more than treating it early.

Recovery and aftercare

Most people go right back to work or their day.

  • Numbness wears off in a few hours. Wait to eat until it does, so you don't bite your cheek.
  • Mild soreness for a day or two is normal. An over-the-counter pain reliever, taken as the label directs, usually helps.
  • Sensitivity to cold can happen for a week or two as the gums heal. A sensitivity toothpaste can help.
  • Rinse with warm salt water a few times a day for the first couple of days.
  • Keep brushing and flossing, gently. Clean gums heal faster.
  • Skip smoking while you heal, if you can. Smoking slows healing and makes gum disease worse.

Call us at 210-545-3929 if you have swelling, fever or pain that gets worse instead of better.

Keeping your gums healthy afterward

Our office is at 14400 Jones Maltsberger Rd, Suite 101, San Antonio, TX 78247, close to Stone Oak, Hollywood Park, Hill Country Village, Castle Hills, Alamo Heights and Live Oak, so regular gum-care visits are easy to fit into your routine.

A deep cleaning gets gum disease under control, but the bacteria can come back. That's why, after a deep cleaning, regular cleanings are replaced with periodontal maintenance visits. These are gum-care cleanings that keep the pockets clean below the gumline.

Many patients need periodontal maintenance every 3 to 4 months, at least at first. At each visit we re-measure your pockets so you can see your progress. With good home care, many pockets shrink and gums stay firm and pink.

Our Serenity Gum Care membership includes 2 periodontal maintenance visits a year, and members get 15% off any extra visits they need.

Common questions about deep cleanings

How long does a deep cleaning take?

Usually about 45 minutes to an hour for every two quadrants. Most patients have the full mouth done over two visits.

Can gum disease be reversed?

The earliest stage, gingivitis, can usually be reversed with a professional cleaning and better brushing and flossing. Once it becomes periodontitis, bone loss can't be fully undone, but a deep cleaning and regular maintenance can stop it from getting worse and help you keep your teeth.

Can I just get a regular cleaning instead?

If you have deep pockets and tartar below the gums, a regular cleaning only cleans the surface and leaves the infection underneath. It can make your teeth look clean while gum disease keeps getting worse. We'll show you your measurements and X-rays so you can see why we recommend it.

Will my teeth feel loose after a deep cleaning?

Teeth can feel a little different as the swelling goes down and the tartar is gone. As gums heal and tighten, many patients find their teeth feel firmer over the following weeks.

Does insurance cover a deep cleaning?

Most dental PPO plans cover scaling and root planing at about 50% to 80% after the deductible. We'll check your benefits and tell you your share before we start.

How much is a deep cleaning without insurance in San Antonio?

In San Antonio and nationally, scaling and root planing commonly costs about $200 to $400 per quadrant without insurance. We'll give you a written estimate before treatment. With our Serenity Gum Care membership ($399 a year), you get 15% off your deep cleaning.
